PHOTO AND MEDIA PERMISSION

By selecting “I Agree,” I give First Baptist Church Rowlett (“FBC Rowlett”) permission to photograph, record, and use the image, likeness, or voice of me and/or the minor child(ren) included in this registration while attending Fall Festival. These images or recordings may be used in church publications, promotional materials, displays, social media, websites, advertisements, or other media related to FBC Rowlett and its ministries. I understand that I will not receive compensation for such use.

EMERGENCY MEDICAL AUTHORIZATION

I authorize FBC Rowlett staff or designated representatives to obtain reasonable emergency medical care for me and/or the minor child(ren) included in this registration if an illness or injury occurs during Fall Festival and I cannot be reached or am unable to provide consent. I understand that FBC Rowlett does not provide health or accident insurance for participants and that I am responsible for all expenses associated with medical care or treatment.

ACKNOWLEDGMENT OF RISK AND RELEASE OF LIABILITY

I understand that Fall Festival is a large, crowded public event that includes carnival-style rides, games, inflatables or other attractions, trunk-or-treat activities, food, candy, pedestrian and vehicle traffic, and other recreational activities. Participation may involve risks, including falls, collisions, allergic reactions, illness, property damage, serious injury, or other unforeseen circumstances.

I voluntarily choose to attend and permit the minor child(ren) included in this registration to participate. I accept the risks ordinarily associated with attendance and participation and agree to follow all posted rules and instructions. I understand that I am responsible for supervising my child(ren) throughout the event.

To the fullest extent permitted by Texas law, I release and hold harmless First Baptist Church Rowlett and its pastors, employees, volunteers, representatives, and agents from claims arising from ordinary negligence connected with attendance at or participation in Fall Festival. This release does not apply to gross negligence, reckless conduct, or intentional misconduct and does not waive any rights that cannot legally be waived.

By selecting “I Agree” and submitting this registration, I confirm that I am at least 18 years old; that I am registering for myself and/or am the parent or legal guardian authorized to register the listed minor child(ren); and that I have read, understand, and agree to the terms above.
